Drawing on an evidence base predominantly from the USA and Scandinavia, early health coaching innovators have been working from the bottom up to change the way health and care professionals communicate with patients and clients and to demonstrate the positive impact of a health coaching approach.

The perseverance is paying off, and personalised care is now central to NHS England's Long Term Plan which includes health coaching as an effective way to improve self-management. In order to maximise impact, areas that have been successful in implementing health coaching locally need to share their stories to continue building momentum and to support others beginning their adoption journey.

This report explores four case studies of successful health coaching adoption, adaptation and embedding, and explores the impact they report has resulted from health coaching.
